I havn't tried yet, but is it possible to make custom backgrounds in photoshop? If it is, you could make an awesome personalised birthday background.

You can create your own backgrounds.
If you look at the link I posted for the DVD-A themes, there is a specification for how to make themes. Look in that one for the specifics regarding the background image itself.

The DVD-A forum would be able to provide support on the subject as well.

Any image can be a menu background - I do it all the time. My favorite is to use photos which have had some of the more artistic Photoshop actions applied to them (ie, make the photo look colored with chalk, watercolor, pencil, etc).

I then will drop the opacity way down on that layer so that the image is faint, and much less of a distraction to the menu elements. I save the file as a PNG file, and use those as menu backgrounds.
