The "Fault Module Name" is CFHD.DLL which is the Cineform VFW codec. Are you using any Cineform files in the project?

no cineform clips ...
have tried sony 10bit YUV , blackmagic RGB , sheer RGB & YUV , and the file that Vegas renders out under using the blu ray template.

Speaking from logic... After a backup, or at least a restore point, what happens when you remove cineform -- get it out of the picture?

I've found all sorts of problems in the past, sometimes from just having cineform installed... & I also grew to detest (if not out & out loath) it. IOW I'm too biased, & John would probably know best when it comes to getting things working with cineform intact, but even having said that the next step logically would be to get cineform out of the way... if it works you not only get done, but you're a step further along troubleshooting.

i removed cineform ...
it still will not prepare .. now pops up below problem
i can burn blu ray in Vegas 9 (1920x1080/24p )
i see Vegas 9 renders a ISO image ( when you burn direct to blu ray) ... but can't take that ISO into DVDa to build a menu based Blu Ray ...

well turns out to be CINEFORM !!

i could have sworn i removed it ...
i uninstalled DVDA5 ... then reinstalled it ...
tried to prepare a Blu ray and the Cineform .DLL popped up again fault module ...
removed cineform = DVDA blu ray prepare worked ...
installed Cineform player 4 = DVDA5b blu ray prepare stopped working ...
uninstalled cineform = DVDA works preparing blu ray files ...

I've been down this road as well.
DVDa5b and Cineform don't play nice together. Heck, I'm beginning to think that cineform and ANYTHING doesn't play nice together.

Keep cineform uninstalled and dvda will work fine.
