From the little I know of this Patrick is correct. Even for broadcasters handling content with embedded ac3 Surround seems to be not common and expensive. I get the feeling this is where Dolby gets their money from the pointy end of the game. Broadcast ac3 metering bridges and the like cost big time.
Don't take this as gospel, just stuff I hear around the bunkers.

You could import Mpeg-2 HD and your AC3 file into Womble Mpeg and export to a .mpg file but you wont get a transport stream I believe.

I use TMPGEnc MPEG Editor. It has muxing and de-muxing tools. The resultant MPEG-2 stream with AC-3 5.1 won't be HDV (.m2t) compliant but it is generic .mpg compliant - they work on my DVD player and PC software decoders. Guess it depends on what you are trying to achieve.
