Acrobat can help you with this, or import as an image to Word, and put text boxes over the image.
I'm sure there are lots of other ways, but my only experience is with Acrobat 3D and Word.

If this is a one time thing, just use a program like PHOTOSHOP to scan the image and use the text tool to fill in the blanks.

I just tried word 2000 and made a mess. Have to do maybe 120 different data on the same form. (120 printed forms)

I had a program a decade ago that was an add-on to the Visioneer scanning software that came with an HP Scanner. It worked really well, and did exactly what you want. You could look at the software portion of their site:

Another place is CutePDF. I have used several of their products and have been reasonably happy. I just looked, and they have a form fill-in product:

Acrobat can probably do what you want as well, but it is costly and, like most things from Adobe, a program that "takes over" your PC not only in the size of its installation, but also in the way it sets up associations, registry entries, startup programs, etc. I have never cared for their products because their designers seem to believe that it is their right to treat YOUR computer as THEIR computer. By contrast, I believe they should treat my PC as I would hope a guest would treat my home. We just had some guests leave, and stuff was strewn about every room, and we had to clean and straighten the whole darn house, so this metaphor is fresh in my mind.

I ended up with Omniform from Nuance for $99.00 which works just like I want. It scans the form and you can apply all of the permanent fill-in blocks and also save repeated info to a user name.
