Menu DVD won't Playback right on PC, hifi OK

Question 1:

I have been making Menu based DVD movies of live Music shows for a while now.
My DVD movies are single Mpeg-2 files built in Vegas 5 with chapter markers added and all intro/outro texts finished.

First thing I do in DVDA is to right click the file and click on ADD MEDIA. AFter that, I try to "insert scene selection menu", and often, it won't work. The new pages form, but it will not create a link button on the main menu page. If I delete the scenes and try it again and again, eventually it works. But it's frustrating. What might I be missing?

Question 2:

Finished DVDs play well on my home DVD player thru my entertainment center, but WILL NOT play correctly using Power DVD 6 or WinXP Media Player on either of my PCs. I can PLAY ALL , but if I try to go to scene select menu (or any other menu) from main menu, I get a "The DVD disc might have been damaged so as to inhibit playback operation." message, and I have to close the player. I get the same message if I am watching the "play all" video and I right click and click on MENU button.

When I am PREVIEWING the DVD in DVDA 2.0 everthing works fine, I can go anywhere. Again...what am I missing?
