Several times I've noticed for no apparent reason several thumbnails disappear and DVD-A claims 'offline'. All the video files are on 'regular' drives and I do not have any power management setting in Windows that spins down the drives. Leaving the application, like I did just now to write this then returning to DVD-A restores the thumbnails without me doing anything else. Another bug apparently. :-(
