It could be a couple of things.

What are your project properties set to? You'll want them to be set ideally the same specs as your video.

Second, what are your quality settings set to in your Preview window? (It's set on a drop-down menu at the top center of the Preview window.)

If they are set to Auto and you're seeing a very low quality playback, it could be an indication that the programs doesn't feel your system is powerful enough to play your video smoothly and full quality.

Hi Steve

ok.. to be clear , when i view in the trimmer window it is in focus and when i use the main preview window it is not. The project settings are the same as the video.The quality settings are auto. I have just tried preview at full and half and the focus is back to nornal. I have 4 gigs ram and a Geforce Gt240 Graphics card. Time to upgrade the card ?

The video card probably doesn't make any difference at all. What you need is a lot of CPU power.

With lower CPUs you have a tradeoff between smooth preview and sharp detail. When i'm editing on a slower computer i find myself often switching back and forth between Preview/Half when i want full frame rate smooth playback, and Good/Full when i want to see details clearly.

Just to be clear, Steve and Chien hit it on the money. For a clear preview, you will need to set the Preview window to "Best" and via a right click on the preview window, unclick, "adjust size and quality.." You will then get your nice, sharp preview. But to keep up with real time, VMS may not preview all of the frames unless you have the latest cpu.
