It's a WINDOWS setting. When you plug in a device, it always pops that window up asking what you want it to do. At some point, you've chosen the open Premiere option AND you told it to "always do this".

That is normally true, but that window has never popped up with my Win 7 setup. And, I would NEVER tell anything to start Premier :)
My HV20, connected the exact same way, has no problem, the system does nothing at all other than give me the "connected" beep. No popup window.

It's probably a bug, but either you have to fix it from that dialog box (accessed from the Win 7 Control Panel\Hardware and Sound\AutoPlay) or you have to go hunting around in the Windows registry with regedit.exe and find the Autoplay entry for the XH-A1. Sometimes programs like GSpot and Filmerit, that give you a summary of all the DirectShow filters, will list the device Autoplay handlers, but that will only give you a hint as to where the registry entries are located. It's usually located at:

[HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Explorer\AutoplayHandlers\UserChosenExecuteHandlers\VideoCameraDeviceHandler]
@="MSPromptEachTime"

The last item indicates that Windows will prompt you every time the video camera is plugged in to the firewire port.

If you're feeling really lucky, you can use regedit and delete this entry:

[HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\Explorer\AutoplayHandlers\Handlers\AdobePremiereProCS3CameraArrival]

(or CS4, if that's the version that you have). Registry editing is dangerous, so don't do this unless you a) make a Win 7 restore point first and b) are completely comfortable directly editing the Windows registry. Don't blame me if you edit the registry and your PC won't boot.

OK, how weird is this?
I just did some more testing, if I have an HDV tape that was recorded to in SD, I get the Premier start, and really bad capture once I shut Premier down and start vidcap. BUT, if I have an HDV tape with HDV recorded to it, Premier does NOT start, and capture is perfect.
I also reinstalled Vegas 8.c, exact same results.
Does this make sense to anyone?
(I accidentally recorded SD onto one of my Canon HDV master tapes)

Yes, that makes sense. Open Premiere and check to see what the default capture options are set to. SD & HDV captures via Firewire are handled differently by the OS. As an aside, this issue is a common cause of confusion, when people try to capture HDV and find that the video camera is "not recognized" by the software. Usually, the cause is that the software is in SD capture mode.

Thanks jabloomf1230. On the capture thing, I was using the correct capture mode, I knew it was sd, but what really confuses me is how, now this is strange, my system seemed to be able to determine what mode was on the tape. Steps: With the camera off, plugged in firewire, turned camera on, SD recording in the camera, Premier starts up; camera off, connect firewire, HDV tape in the camera, nothing starts automatically.
Hopefully I won't have this issue in the future, I normally record everything in HDV, and if necessary render out in SD. The HDV tapes are too expensive to not record HDV, I had changed the camera to SD mode for on quick shoot, and forgot to set it back before I started the next shoot.
Thanks again, I'll look at Premier and see if I can find the option, even the menus in Premier are LOTS less intuitive than Vegas.

Check the Autoplay settings in the Windows Control Panel.
