The SONY DV codec (actually developed by SonicFoundry) is only used when rendering to DV .avi.

I'm too busy burning discs right now to check, but once you've gotten into the MPEG4 template, can't you click the Custom button, go to the Video tab, and choose whatever frame size you wish?

Correct.

For SD (standard definition) video, yes. For lower/higher resolution, change presets as desired.

If you are going to view the MP4 file on a computer, then the size should be set for 640x480 or some multiple of a 1.333:1 aspect ratio, like 160x120, 320x240, 512x384 etc.)
