Generally, people use a frame from the video they're linking to. Check the Thumbnail section of DVDA's Help to see how to do that.

Personally, I generally either make my own button images (not thumbnails, really, since a thumbnail is, by definition, just a smaller version of the larger picture) or I use text buttons.

Neither. I use Paint Shop Pro.

you can layout all the elements in Vegas, and then export them as individual files.
you have the ability to see everything exactly as it will appear on your disc, including moving backgrounds, and/or buttons.
