Worksheet to keep track of editing sessions

Does anyone know where I could get a worksheet or template for tracking the steps involved in an editing session?

I am starting work on a video that will showcase our department to other employees in the corporation. Some of what I want to do will involve building up layers of video, render it, add more layers, render it, etc. until I get the final effect I want.

What I would like is a worksheet that I can record the different steps (clips, effects, lengths, audio, whatever) that I can keep in my records. I will be keeping all of these steps in individual projects, but sometimes it is easier just to look up on a worksheet a particular step if I need to duplicate it in another project.

Also, is there a proper name for this type of worksheet?

I haven't seen a worksheet specifically for this kind of thing. But, for what it's worth, I recall reading that Vegas (as in Vegas Vegas, not Vegas Movie Studio) will record all edits made in some kind of text format. I think it's for repeatability, but would be interesting for this kind of thing.
