Is your background menu the same pixel size as your project size?
The dvd setting is 704x576. The background image is 1920x1080. However the thumbnails are not part of the background image. They are the buttons made by the software when creating the scene selection menu.
EDIT: I just adjusted the background image and ran out anew copy and it was the same issue. I also did another with a default background image from the software and it did the same thing again.

But it may be relevant as the highlighting is probably positioned relative to the background.
Anyway have a look at the two old threads below to see if there's anything there that relates to your problem. You are taking us back a few years with 704x576 instead of the nowadays normal 720x576.
I use 704 for this because the original supplied by my client is 1280x720 (16:9) and if I use 720 It adds small black bars to the left and right of the video.
The highlighting is positioned relative to the thumbnails as it is all apart of the button.
That's because SD widescreen 16:9 and SD widescreen "16:9" aren't the same. SD is actually slightly wider hence the small black bars when you convert HD to SD and bars at the top and bottom when converting SD to HD format.
1280/720 = 1.7777 720/576 with PAR 1.4568 = 1.8210 Note the difference.
I wouldn't expect using 704 instead of 720 to make any difference to the image shape as 704 uses a different PAR (pixel aspect ratio). If somehow you are using 704 with 720s PAR maybe it is causing problems.
There may be a better way depending on which DVDA version you have. What is your version and build number?

Make the project 720x576 16:9 Double click on each thumb so it opens the media, right click on it and select "Crop & Adjust" Right click on the Crop box and select "Match Output Aspect". You will note it crops a small amount top and bottom to make it fit SD WS Make the background in Photo shop 1049x576 pixels. That's the square pixel equivalent of SD WD.

I changed various setting you mentioned 1 by 1 to find out what triggers the issue. The entire problem is the "Match Output Aspect" Even with a large 1920x1080 background image as long as you click crop and adjust and match output aspect on the media file it will fix the alignment of the highlights.
It confirms my suspicion that the highlighting, which is in the form of a mask, doesn't attach its position to the thumbnail. Instead it somehow independently relates its position to the background image.
