Motion Blur

Does any one no how to create an effect like this? Is it done using the motion blur effect or is it camera work done by the lens? Check sample #2 on this website http://www.mgxproductions.com/wedding.htm

Looks like a cookie cutter, center oval and two layers of video. I would say linear blur applied to background. Try this
