OT - Where to get release forms?

I'm going to do a video for a pediatric therapy clinic. It will involve shooting the therapist, staff, but also the child patients.

Where would I get the release forms for something like this? The parents of the child patients would obviously be the ones to sign it, but I want to make sure I'm covered.

I hope to make nearly as good as the one jrazz recently did, showing the outside of the building, and some shots inside of the the kids getting therapy.

And would the release form also cover any HIPAA regulations?

jrazz - did you have to do anything special to video the dental patients?

Go to the VideoUniversity site and you'll find generic release forms for amateur & professional talent (including minors) as well as Materials and Location releases.

edit: the reason for paying amateurs the token sum of $1.00 is that it makes it a completely legal release. That is, they did the work and got paid for it so they can't come back on you or your company in the future saying "I didn't get paid so I'm going to sue you".
