what are you shooting? The reason i ask is there are different releases for different things. Are your subjects under 18? Over 18? Or is your subject a business with a notable trademark? As far as I know, there isnt just a general blanket release. keep in mind that there are different releases for different things. As an example, a minor release differs from an adult release because the minor's guardian has to sign.

just search release forms , plenty out there to get you started, however as with any document would be wise to have a lawyer understand exactly what you are shooting and draw up the form, very small price to pay for comfort.
