Anybody experiencing problems with multicam editing in V9?

I'm editing a 4 camera shoot (converted to 1920x1080 cineform) and I'm periodically getting clips that go out of sync. I'm using the Ctrl+D shortcut to get crossfades when switching cameras and the out of sync is usually about the length of of the crossfade. When I go back to the orginal line up of cameras they are in sync.

I'm also being careful to not have my ripple mode engaged.
