It means you care as in sympathy or wanting to give a hug to someone who is sad or in need. The character is holding a heart.

I've got it in a few FB comment replies from women but I couldn't find it on FB, it's not in the right click Emoji & it's not on my Samsung phone, so it might be an iPhone emoji?
